For Prescribers

Access formulary, prior authorization forms, and clinical criteria:

• Visit the Navitus website
• Select “Prescribers”, then click “Prior Authorization”
• Enter your NPI number and state to access the prescriber portal

For Pharmacies

Access payer sheets and pharmacy tools:

• Visit the Navitus website
• Select “Pharmacies”, then click “Pharmacy Login”
• Enter your NPI number and NCPDP number to access the pharmacy portal
