How do I know what services are covered by my plan?

You can find out what services are covered by your Align Senior Care plan, as well as included benefits, by reviewing your plan’s Summary of Benefits. For even more detail, read your plan’s Evidence of Coverage. Both documents can be found online under Member Plan Documents.

Can I still visit my current doctors?

Align Senior Care has a network of providers that are covered by the plan. If your provider is in-network, you can continue to visit them uninterrupted. If your provider is out-of-network, you may be responsible for the costs associated with receiving care from them. To check if your providers are in-network, please visit our Search Our Network page.

Will Align Senior Care cover my prescriptions?

Align Senior Care includes Part D prescription drug coverage for members. Coverage varies based on the type of drug prescribed. Each year, a list of covered drugs (called a formulary or drug list) will be published with information about covered prescriptions, coverage rules, and limitations. To search for covered drugs, visit our Search Our Network page.

You can find a complete list of your plan’s provider and pharmacy directory online under Plan Documents . You will find a PDF version of the Pharmacy Directory, Provider Directory, and Drug Formulary here.

Will my Align Senior Care coverage rules ever change?

Your coverage rules under Align Senior Care are subject to change annually. Each year, you will receive your Annual Notice of Change (ANOC) letter notating the year over year changes. In addition, the plan’s Evidence of Coverage (EOC) will be available online to view in the Plan Document section of our website. It is important that you review these documents.
