If you are a current Align Senior Care member, you can log in to your Member Portal by visiting https://secure.healthx.com/AlignSeniorCare.Member.

The Member Portal offers members secure and HIPPA approved web-based access to their health care information, including claims, eligibility, and benefits.

The Member Portal allows you to:

• Verify Member eligibility
• View Member authorization history
• Check the status of claims/encounter information
• Submit Direct Member Reimbursement requests
• Change your PCP
• Request a Member ID Card

Are you a caregiver or guardian?

To gain access to the member portal on behalf of a member, you must be at least 18 years of age and have legal authority to act on the member’s behalf.

If you are a legal representative (e.g., Power of Attorney or Legal Guardian), a copy of the supporting legal documentation (e.g., POA or guardianship papers) must be submitted to the Enrollment Department for verification. If you have not previously submitted this documentation, please email it to the Enrollment Department at [email protected].

Please Note: HIPAA Authorization and Health Care Proxy forms do not provide authority to act on the member’s behalf.
