Your central point of care

With an Align Senior Care nurse practitioner and Care Ally, you’ll have a trusted care team to simplify communication, plan appointments, support transitions, and keep your care aligned.

Your nurse practitioner will:

Having a Care Ally is like having your own private medicate advocate! Align Senior Care made it easy to order over-the counter items. We were given a special debit card which paid for all these extra necessities that regular Medicare does nto pay for. I did not have to worry about parts A, B, and D since everything comes as one statement. And, if my mother needs any medical needs, like UTI screenings or just a checkup, they will come to her. Align Senior Care suits the needs of aging parents.

I am happy with my current health plan. Why would I enroll in Align Senior Care?

We are so glad that you have a plan you trust. Align Senior Care offers a unique blend of clinical care and Medicare Part C coverage that allows our members to have additional touchpoints with medical providers—often in the comfort of their own community. Align Senior Care will not replace your existing care team—we work with them to provide additional support on your health journey.

Who is part of the Align Senior Care team?

Our team consists of nurse practitioners and Care Allies. Members receive care services from our providers within your community and can support you in appointment planning, benefit usage, and more.

I have Original Medicare and a Medicare supplement. What happens if I enroll in Align Senior Care?

We will become the primary payer of Medicare-covered services, including any prescription drugs you may take for all enrollees.

Do I get to keep Medicare Part B?

Yes! You must be enrolled in both Medicare Part A and Part B to be eligible to enroll in a Medicare Special Needs plan like Align Senior Care. You must continue to pay your Medicare Part B monthly premium to stay enrolled in Align Senior Care.

Can I still visit my current primary care provider?

Align Senior Care has a network of providers that are covered by the plan. If your provider is in-network, you can continue to visit them uninterrupted. If your provider is out-of-network, you may be responsible for the costs associated with receiving care from them.

Where can I check if my provider is in-network?

Will Align Senior Care cover my prescriptions?

Align Senior Care includes Part D prescription drug coverage for members. Coverage varies based on the type of drug prescribed. Each year, a list of covered drugs (called a formulary or drug list) will be published with information about covered prescriptions, coverage rules and limitations.

Who can enroll in Align Senior Care?

Future members must live in our covered service area and be enrolled in Medicare Part A and Part B. There may be additional requirements based on plan type. Schedule an appointment to learn more.
