Align Senior Care is an HMO I-SNP, HMO-POS I-SNP, and HMO with a Medicare contract. Enrollment in Align Senior Care plans depends on contract renewal. Other providers are available in our network. Out-of-network/non-contracted providers are under no obligation to treat Align Senior Care members, except in emergency situations.

Some of the benefits mentioned are part of a special supplemental program for the chronically ill who are diagnosed with cancer, diabetes, cardiovascular disease, autoimmune disorder, dementia, and many other conditions. Not all members may qualify to use these benefits.
