Medication Therapy Management Program

If you have Medicare drug coverage and take several medications, you may qualify for our Medication Therapy Management (MTM) Program.

The MTM program is a free and voluntary service offered by Align Senior Care. If you qualify, you will be automatically enrolled, but you can choose whether or not to participate.

  • There is no cost to you
  • It is a separate service from your pharmacy benefit

The MTM program helps make sure:

  • Your medications are working the way they should
  • Your medications are safe to take together
  • You are using your medications the right way

This program is required by the Centers for Medicare and Medicaid Services (CMS) and designed to improve medication use and reduce the risk of problems or side effects.

How do I participate?

You must meet certain requirements to qualify. If you qualify:

  • We will automatically enroll you in the program
  • We will send you a letter with details and next steps

Want to stay enrolled but decline a service? You can decline individual services (like a medication review) without leaving the program

No longer wish to participate in MTM?

Choosing not to participate does not affect your Medicare drug coverage.

  • You can opt out at any time
  • To opt out, call Member Services and ask to be removed from the program
  • Once you opt out, you won’t receive MTM services or outreach for the rest of the year
Who qualifies for the MTM program?

You may qualify if you either option 1 or option 2:

Option 1: Chronic conditions, medications and cost program

You must meet all of these criteria:

  1. Have at least three of these conditions:

    • Alzheimer’s Disease
    • Bone Disease – Arthritis (including osteoporosis, osteoarthritis, and rheumatoid arthritis)
    • Chronic congestive heart failure (CHF)
    • Diabetes
    • Dyslipidemia
    • End-Stage Renal Disease (ESRD)
    • Human Immunodeficiency Virus/Acquired Immunodeficiency Syndrome (HIV/AIDS)
    • Hypertension
    • Mental health conditions (including depression, schizophrenia, bipolar disorder, or other chronic/disabling conditions)
    • Respiratory disease (asthma, chronic obstructive pulmonary disease [COPD], or other chronic lung disorders)

AND

  1. Take at least eight covered Medicare Part D medications

AND

  1. Are likely to have more than $1,276 in annual (2026) covered Medicare Part D drug costs

Option 2: At-Risk Beneficiary (ARB) program

You qualify if you are enrolled in the Align Senior Care Drug Management Program as an At-Risk Beneficiary (ARB). This program helps members safely use,opioids and other medications that may be frequently misused.

What type of reviews are included?

The MTM program includes two main types of reviews:

Targeted Medication Review (TMR)

  • Happens at least every 3 months
  • We review your medications to look for possible problems
  • We may contact your doctor if needed

Comprehensive Medication Review (CMR)

  • At least once each year
  • A clinician or other qualified health professional will review all of your medications with you

This service is provided confidentially on behalf of Align Senior Care by a Navitus Clinical Engagement Center clinician

How are reviews completed?

The review takes about 20–30 minutes.

Your review is usually completed by phone, but may also be provided in person at your:

  • Provider’s office
  • Pharmacy
  • Long-term care facility

What do I receive after my review?

After your Comprehensive Medication Review, you will receive a written summary of the CMR, which can be helpful to talk with your doctors. It includes:

  • Recommended To-Do-List (TDL) (also known as Medication Action Plan (MAP)): The TDL has steps you should take to help you get the best results from your medications
  • Personal Medication List (PML): The PML will help you and your health care providers keep track of your medications. It explains how to use your medications the right way. Download blank copies of the PML below:
